  1. THREE PROOFS OF RESIDENCY:<\/strong><\/span><\/li>\n<\/ol>\n

    *1st:<\/strong>\u00a0Must be a Mortgage Statement, Deed, signed closing documents, or signed lease agreement.<\/p>\n

    *2nd:<\/strong>\u00a0<\/strong>Must be a major utility bill (ie: JEA, TECO, Internet) dated within 30 days of registration date.<\/p>\n

    *3rd:<\/strong>\u00a0An additional utility bill, homeowner\/renters insurance, vehicle registration, credit card statement or bank state. All proofs must be dated within 30 days of registration date.<\/p>\n

    1. ORIGINAL BIRTH CERTIFICATE<\/strong><\/span><\/li>\n<\/ol>\n

      1. COMPLETED FLORIDA IMMUNIZATION FL680 FORM
        \n<\/strong>STATE LAW FL680 \u2013 NO OUT OF STATE IMMUNIZATIONS WILL BE ACCEPTED.<\/strong><\/span><\/li>\n<\/ol>\n
